Referrals can be issued by any registered health professional such as: your GP, consultant, physiotherapist, dietician and health advisors. Referrals must be issued on the dedicated form opposite which should be familiar to your referring Health Professional. If not please feel free to download the version shown.<\/p>\n

Once the form is completed, we ask that it is either delivered to the Centre or sent via email to the address below for verification. You will then be placed onto our waiting list and contacted for a triage assessment. Once a space becomes available, you will then be contacted to arrange your Initial Assessment with your allocated instructor.<\/p>\n
